Bookstore Placement Agreement
This service is free for all books published with Revelade Publishing LLC. For books published with another publishing company, there is a $10.00 annual fee for each title. For more information: Read the BookStore Placement Agreement
To begin placing books in Revelade Publishing's store:
- Print the BookStore Placement Forms - Click Here for Forms
- Read the agreement
- Print your name and sign and date at the bottom of the agreement
- Fill out the Contact Information page
- Fill out the W-9 form. why
- Mail all pages to:

Once we receive the agreement, you will receive an email with submission and/or payment instructions. (If you'd like to mail payment with the forms, please make check payable to Revelade Publishing, LLC in the amount of $10.00)
To place additional books (once you've signed the BookStore Placement Agreement):
- Fill out the BookStore Placement Information form and submit by online form or mail in form
- If submitting form online, make sure to place the order for your books first.
